SUMMARY:
Under general supervision , provides one-on-one education and nursing services to clients across the age spectrum, both in home and at clinic. Teaches, provides resource referrals, and conducts follow-up as needed.


PRIMARY
DUTIES AND RESPONSIBILITIES:

  • Provides nursing services in clinic and home setting, making referrals to other resources as needed, and following up as needed.
  • Performs nursing assessment, diagnosis, treatment, and evaluation of clients across the age spectrum.
  • Teaches individuals and groups in health promotion and disease prevention.
  • Creates charts and documents recording details of client visits; manages patient case load; maintains file and filing system.
  • Performs administrative duties; including data collection and entry and timely response to phone calls and emails.
  • Refers clients to other community resources; works with other providers to reduce barriers to care for the community.
  • Participates in health clinics and educational programs with clients as required.
  • Participates in team responses as required for epidemiological and all hazards situations.
  • Supports the relationship between the Teton County staff and the general public by demonstrating courteous and cooperative behavior when interacting with visitors and County staff; maintains confidentiality of work-related issues and County information; performs other duties as required or assigned.

MINIMUM QUALIFICATIONS:


Education and Experience:

  • Associate’s Degree in Nursing; AND two years of experience; OR an equivalent combination of education, training, and experience.

Required Licenses and Certifications:

  • Must possess a valid Wyoming/Idaho Driver’s License and maintain an acceptable driving record.
  • State of Wyoming Nursing License.
  • Cardio Pulmonary Resuscitation (CPR) Certification.

Required Knowledge of:

  • Federal, state, and local laws pertaining to health care and public health.
  • Health Insurance Portability and Accountability Act (HIPAA) laws and regulations.
  • Practices, principles and methods of professional nursing.
  • Safety rules and regulations, occupational hazards, and safety precautions.
  • Principles and procedures of recordkeeping, reporting and retention.

Required Skills in:

  • Operating a personal computer utilizing standard software and some specialized software; operating standard office equipment.
  • Providing professional nursing and health care services.
  • Establishing and maintaining cooperative working relationships with coworkers and other individuals in contact during the course of work; communicating clearly and concisely

Physical Demands/Work Environment:

  • Work is performed in County clinics as well as in patient homes; may be exposed to adverse conditions and weather during home visits. This position has an extreme exposure risk to biohazards associated with blood borne and airborne pathogens; the use of proper protective equipment and clothing is required. Inoculation may be required.
